Reasonable Accommodation Packet Request

Employees who are unable to perform one or more essential functions of their position due to a disability and/or other health-related condition (work-related or non-work-related) may request a reasonable accommodation packet from Human Resources. Additionally, employees who are unable to return to work upon exhaustion of family medical leave entitlements may request a reasonable accommodation packet.
