Overview
Use this service to request a computer purchase outside the normal annual purchasing cycle (which occurs near the start of each fiscal year). Please only request this service if
1) You are requesting a computer for a position that was newly added mid-fiscal, and you do not have another computer that could be assigned.
2) You need to replace a workstation has experienced hardware failure and is out of warranty (e.g. a non-domain computer has broken.) Use this request only when work cannot continue until new hardware arrives.
Description of service
This service covers rare new-add computers and one-for-one replacement of desktops or laptops that have experienced critical hardware failure (e.g., motherboard, disk, or power) and are out of warranty or not economical to repair. IT will:

Confirm failure through troubleshooting and warranty status checks

Select an in-stock spare of equivalent specification or place an expedited order with approved vendors

Image and configure the new device when it arrives
Note: Requests for hardware upgrades, refreshes, or extra devices (e.g., a second laptop) must wait for the normal fiscal-year bulk purchasing cycle and should not be submitted through this service.
Before submitting this request

Verify that standard troubleshooting has confirmed a non-recoverable hardware failure.

Obtain supervisor approval and confirm that no loaner equipment will suffice temporarily.

Gather the following details:

Asset tag or serial number of the failed device

User name, department, and location (if applicable, e.g. Station 6-2)

Description of the failure and any error codes

Peripheral requirements (docking station, dual monitors, specialty adapters)

Date the failure was first reported and any impacts to operations

(If applicable) ID or link to the IT troubleshooting ticket confirming the hardware has failed

Once we receive your request, we will assign it to a technician who will contact you during normal business hours.
