Employee Name/Address Change Request

Before submitting this request: 

Please note: A change in name and/or marital status will require a new Social Security Card and/or supporting documentation (i.e. a marriage license or divorce decree). Such documentation may be brought to the Human Resources office or submitted by email to: borough.human.resources@matsugov.us

PLEASE DO NOT INCLUDE DATES OF BIRTH OR SOCIAL SECURITY NUMBERS IN THIS TICKET.

How we process this request:

Human Resources updates the employee's personnel file and insurance, if applicable, and notifies Payroll.
