Please Note: The purpose of the reasonable accommodation packet is to provide the employee with the forms necessary to assist the Matanuska-Susitna Borough in determining whether, or to what extent, a reasonable accommodation is appropriate for an employee with a disability to perform one or more essential functions of their job safely and effectively. The employee has the responsibility to ensure that the medical provider follows through on requests for medical information.
How we process this request:
1. The employee is to complete the attached Matanuska-Susitna Borough Employee Request for Reasonable Accommodation form and submit directly to Human Resources.
2. The employee is to complete and sign the Health Care Provider Release Form.
3. The employee is to give their health care provider(s) the following documents: Current job description; Health Care Provider Release Form; and, the Health Care Provider Verification Form.
4. The employee is to provide Human Resources with the Health Care Provider Verification Form once completed by their health care provider.
5. As part of any interactive process, the Matanuska-Susitna Borough Division of Human Resources will use information obtained from the employee and the health care provider to identify potential reasonable accommodations. The Division of Human Resources is not required to accept a recommendation from a health care provider and maintains the ultimate discretion to identify a reasonable accommodation that allows that employee to perform the essential functions of the position.
