Before submitting this request:
Be prepared to provide the following information for the occupational health referral request: name of employee to be referred, reason for referral, type of referral (vaccination, respiratory fit test medical evaluation, respiratory fit test, audiogram, physical, etc.), and other relevant details will be required.
Note: If requesting a respiratory fit test, the type of mask used (half-face, full-face, etc. will need to be noted).
How we process this request:
Once we receive your request, it will be evaluated to ensure that the requested occupational health testing/screening is appropriate based on the position held. If appropriate, Human Resources/Health and Safety will send a referral to Beacon Occupational Health and a copy will be emailed to the referred employee. To schedule the appointment, the employee will need to contact Beacon Occupational Health directly. They have clinics located in Wasilla and Anchorage. Beacon’s contact information will be included in the confirmation email that is sent to the employee.
