Description of service:
Family Medical Leave guarantees an employee the right to take leave, the right to the continuation of health benefits, and reinstatement to an equivalent or the same position. The FML packet provides the MSB with the necessary information to determine eligibility.
How we process this request:
Hours of work during the last 12 months are calculated to determine Federal (FMLA) and/or Alaska State (AFLA) requirements. If employee meets the hours requirement, an initiation letter and Certificate of Health is sent to employee to be filled out by Doctor. Approval is determined after Certificate of Health is returned from the doctor.

Additional Information:
Eligible employees are entitled to family and medical leave for the reasons set out in state or federal law, which include:
- The birth of a child of the employee, or the placement of a child with the employee for adoption (or, under federal law, for foster care), provided the leave is taken within one (1) year of the birth or placement.
- The need to care for the employee's family member who has a serious health condition.
- The employee's own serious health condition which makes the employee unable to perform their job functions.
- For qualifying exigencies arising out of the fact that the employee's spouse, son, daughter, or parent is on active duty (or have been notified of an impending call to active duty status) in the Armed Forces in a foreign country.
- To care for a FMLA covered service member with a serious injury or illness.
