Please note: A change in name and/or marital status will require a new Social Security Card and/or supporting documentation (i.e. a marriage license or divorce decree). Such documentation may be brought to the Human Resources office or submitted by email to: borough.human.resources@matsugov.us (For your safety, we recommend encrypting all emails that contain private health information.)
PLEASE DO NOT INCLUDE DATES OF BIRTH OR SOCIAL SECURITY NUMBERS IN THIS TICKET.
How we process this request:
Human Resources updates the employee's personnel file and insurance, if applicable, and notifies Payroll.
